As a child's overall health and well-being can directly affect his/her ability to learn, the Pennsylvania Department of Health have set requirements for immunizations and regular health screenings for school-aged children. All kindergarten students are required to have a physical and dental exam before or during their first year of school. Below you will find the links to the Physical and Dental forms, as well as the Pennsylvania Department of Health immunization requirements. If you have not previously uploaded your child's immunization record with you, you must bring a copy with you to your registration appointment. The nurse will then review them and inform you of what, if any further immunizations are needed for your child to start kindergarten.
