Kindergarten Registration

Kindergarten Registration Forms and Instructions

Please complete the enrollment forms online by clicking on the following link: AASD Kindergarten Registration. You will need to complete the enrollment forms prior to your scheduled appointment. If you have any questions regarding the online enrollment process, please call 570-888-7766, Ext. 4104.

You will be required to upload the following items into the online registration portal prior to your appointment:

Your child's birth certificate,
your child's immunization records,
two proofs of your residency in the Athens Area School District,
custody / guardianship paperwork (if applicable)
Residency-Affidavit.

Please bring the child that you are registering to your appointment. At the time of their visit the child will receive vision, hearing and Kindergarten Readiness Assessments.

Kindergarten Health Requirements

As a child's overall health and well-being can directly affect his/her ability to learn, the Pennsylvania Department of Health have set requirements for immunizations and regular health screenings for school-aged children. All kindergarten students are required to have a physical and dental exam before or during their first year of school. Below you will find the links to the Physical and Dental forms, as well as the Pennsylvania Department of Health immunization requirements. If you have not previously uploaded your child's immunization record with you, you must bring a copy with you to your registration appointment. The nurse will then review them and inform you of what, if any further immunizations are needed for your child to start kindergarten.
